What is a health survey?

A health survey is a tool used to gather information on the health behaviors, health challenges, and health needs of people from a selected area.

What will the Community Environmental Health Survey measure?

This survey will:

Gather data on health behaviors, health challenges, and health needs experienced by people within the area your Community-Based Organization serves, while also accounting for the indoor and outdoor environmental conditions that can impact health.

Gather data through questions that focus on human and environmental health within 1) households and 2) broader communities.

How can the findings from the Community Environmental Health Survey be used for advocacy?

  • Inform community members of shared, priority health and environmental concerns to strategically plan and optimize resources for action.
  • Inform public health/elected officials of health concerns in the community.
  • Determine how city, county, state and federal resources might be used for intervention programs.
  • Support legal action if there is a clear association between a harmful exposure/climate impact and a health outcome (disease).
  • *Data will be co-owned by CBOs and we will seek consent before using data in any way.

The main purpose of this initiative is to empower communities with data for advocacy.

What types of information will be collected through the Community Environmental Health Survey?

    • Geographic Information– This documents the city, state, and zip code of the person filling out the questionnaire, and should reflect the location of the household and broader community
    • Individual Demographics– This documents basic demographic information of the person filling out the questionnaire
    • Household Residents Demographics– This documents basic demographic information of those residing in the household
    • Physical and Mental Health of Household Residents
    • Household Environmental Safety
    • Community Health and Environmental Safety
